Referral and Intake Process

Referral Process

  • Anyone can make a referral
  • Call to provide basic demographic information and to schedule Intake appointment
  • Referrals made for children who can be seen in school are accepted immediately



Intake Process

  • Collection of information regarding family  history, demographics, medical issues, need for treatment, past or present substance abuse history, sign releases of information
  • Upon completion of Intake, case will be distributed to a clinician and followed up with a phone call to schedule appointment
