Intake Process

Adult Family Care

Participants are carefully assessed by a registered nurse and case manager and then matched to caregivers based on:

  • Common interests
  • Location
  • Physical suitability of the home
  • Ability of caregiver to provide required care

Once matched both parties are required to meet on 3 separate occasions 

Prior to move-in date, a care plan will be developed with the help of both the participant and caregiver

On-going support and training is provided

Participant
  • Must have or be eligible for MassHealth Standard (private pay is also available)
  • Require health care management due to medical or psycho/social need
  • Be willing to participate
  • Pass background check

Caregiver:

Please note: if eligible, family members can be paid for providing care to another adult family member or friend in their own home. Please call for more information.

  • Must be able to provide required care
  • May not be the spouse or guardian of the participant
  • Pass background check

Home Assistance Program

  • For elders and disabled adults
  • Must live in designated housing units
  • At risk of institutionalization
