Referral and Intake Process

Referral Process

  • Hours of Operation: Monday through Friday 8 am to 5 pm
  • Submit completed referral form

Intake Process

  • Comprehensive assessment with Care Manager to determine eligibility and placement into appropriate programs

Other Elgibility Criteria:
Adult Day Health
  • Must be eligible for basic services
  • Must have medical need (call for details)

Basic Coastline Elderly Home Care Services:

  • Age 60 years or older (exceptions for clients with Alzheimer's diagnosis)
  • Income (sliding fee scale for monthly co-payment* based on income)
  • Needs/limitations assessment (must have a critical unmet need)

* There is no copay for MassHealth participants

Personal Care Attendant Program (PCA):

  • Individual of any age with a debilitating disability
  • Completed assessment of eligibility with a Coastline registered nurse
  • Must qualify for MassHealth

Caregiver Program:

  • Anyone caring for an older adult who suffers from chronic disease, dementia, disability, or other illness
  • Grandparents 55 years or older who is the legal primary caregiver for a grandchild 18 years of age or younger, or an adult child with disabilities
  • Call to determine eligibility

Behavioral Health Services

  • Must meet criteria for basic services
  • Must be eligible for in-home care
Benefit Assistance Programs:
  • Age 60 years and older

Community Programs/Services

  • Most 60 years or older (some 55 years and older)

Money Management Program
