Referral Process

Anyone may refer or request services.

Contact or call the information and referral department at (508) 675-2101.

Intake Process

Intake and screening is completed by phone. Individuals will be referred for services at that time based on needs and eligibility.

Services are available to elders 60 years and older, disabled adults 18 years and older, and their families. Programs are funded in whole, or in part, by contracts with or by grants from the Massachusetts Executive Office of Elder Affairs.
