Referral and Intake Process

Referral Process

  • Referrals accepted from physicians, hospital, skilled nursing facilities and community and community agencies
  • To make a referral, VNA needs the following information:
    Patient's name, address, telephone number, birth date, physician's name, diagnosis and form of health insurance

Intake Process

  • Collect demographic information
  • Patient will be seen within 24 hours of referral
  • Will coordinate all aspects of home and hospice care and work with the patient's physician to ensure that appropriate services are in place
  • Connect patient and family to other needed community resources, such as elder services, Meals-on-Wheels, and pharmacy assistance programs
  • If necessary, facilitate the ordering and delivery of medical equipment and supplies
  • Financial Assistance Services available

  • Have had frequent inpatient psychiatric hospitalizations or emergency room visits
  • Newly diagnosed or require major changes to medication plan
  • Difficulty keeping appointments, following treatment or taking medication appropriately
  • Dual medical and psychiatric diagnosis
  • Have extended wait times between discharge from hospital and follow up appointment
  • Have self-care deficits and are in need of linkages to community services
